Application Process   Discuss the following with your Pastor and/or parish staff or Agency Director: — your parish /agency experience and your commitment to ministry; — the parish/agency’s willingness to sponsor you in the program; — your choice of advisor (download Advisor Information form); — the parish/agency’s financial support and/or sharing of costs. Ask the pastor or agency director and advisor to complete the Recommendation Form (press on Recommendation Form to Open / Print this Form in Adobe Acrobat). Submit the completed application to the Lay Ministry Formation Program office as soon as possible. After receiving your application and sponsoring community’s recommendation form, we will contact you to schedule an interview.
